Complaint Investigation Summary
Report #: 5105
COMPLAINT ID: C00052510 Site Location: 4283 N WILSON WAY
Activity Summary
Activity Date Recorded by
09/16/2020 Met up with Al. Took pictures of the replaced tight line pipe, it was schedule 40. Al AFONSKAIA
poured a gallon of water down the clean out before the replaced pipe area to
check for leaks. There were no leaks. There area was dry, Al said that they had
clean out the leaded sewage. Let him know that I send out another notice for the
other building septic tank since it was located under the spray booth. I let him
know that the tank will have to be destroyed and replaced under permit at time of
service.
10/14/2020 RECEIVED COMPLAINT RETURN MAIL, NOTICE TO ABATE, ON 10/14/2020 BALLWAHN
NOTE: RETURN TO SENDER
UNCLAIMED
UNABLE TO FORWARD
RETURNED TO DARIAAFONSKAIA.
01/21/2022 Review file records. The original complaint regarding the "two holes in the septic GIRARDI
tank, no working restrooms, and tightline caped off" is abated per Log2 report.
However, there is a note regarding an alleged paint booth being built on top of an
existing septic tank. File records revealed that two building permits were issued
BP-0000809 and BP-154891 which were finaled. These booths were building
inside the existing foot print of the building that was approved by UP-78-0035.
There appears to be two existing septic systems on record that are located outside
of the existing footprints.
01/24/2022 Spoke to Al Dibattista 209-401-1432 and he indicated that there is a paint booth GIRARDI
located on the south west corner of the south building (toward the back area away
from Wilson Way) and a Septic Tank is located under an existing paintbooth (built
during the early 1980s). Conducted a document archive search and the there are
no building permits found for this paint booth located outside or the original
building footprint. To refer to CDD for paint boot installation without building
permits which will initiate the building permit process and resolve the issue prior to
issuance of building permit. The original complaint regarding the "two holes in the
septic tank, no working restrooms, and tightline caped off" is abated.
5105.rpt Page 2 of 2
