OBSERVATION:An accurate UST Monitoring Site Plan was not submitted. Site plan only includes three dispensers. Dispensers on site plan show floats and chains. Floats and chains have been removed. REGULATION GUIDANCE:A site plan must be submitted identifying the locations where monitoring will be performed. The site plan must show the general layout of tanks and clearly identify locations of the following equipment,if installed: 1)monitoring panels,2)in-tank liquid level probes(if used for leak detection); 3)devices monitoring tank annular spaces or vault;4)devices monitoring product piping,5)devices monitoring fill piping,6) devices monitoring vent piping; 7)devices monitoring vapor recovery piping;8)devices monitoring vent/transition sumps;9)devices monitoring under-dispenser containment; 10)line leak detectors;and 11)devices monitoring any other secondary containment areas. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a repeat violation,Class 11. 107V MSC 25292.2;CCR 2711(a)(11)Failure to submit and maintain complete and current financial responsibility documents. OBSERVATION:Complete financial responsibility documents have not been submitted to the EHD. The UST Certification of Financial Responsibility was missing name and signature of witness or notary. REGULATION GUIDANCE:Current financial responsibility documents are required to be submitted annually. CORRECTIVE ACTION:This was corrected on site. No further action required. This is a repeat violation,Class IL 109, CCR 2715(a)(1)(B),2715(a)(3)Failure to submit"Designated Underground Storage Tank Operator Identification F Form"within 30 days. OBSERVATION:A new designated operator,Amy Winkler,began conducting inspections on 2/25/2022 and notification was not provided to the EHD within 30 days of the change. REGULATION GUIDANCE:The owner or operator shall submit a"Designated Underground Storage Tank Operator Identification Form"no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION:This was corrected on site. No further action required.
