The following is an itemized list of hazardous materials violations that have not been addressed for BLUE SHIELD OF CALIFORNIA as of September19, 2022, Open violations from March 23, 2022 inspection Violation #6 - Failed to electronically submit a site map with all required content. OBSERVATION The site map is not complete. The site map does not show/address the following required elements: 1 , Internal roads, 2, Storm and sewer drains, 3. The main access and exit points to the structure are not shown, 4. Emergency response equipment (spill control). 5, Evacuation staging area. 6, Emergency shutoffs. 7, The locations of the hand sanitizer. 8. Much of the information on the site map is not legible, REGULATION GUIDANCE: Asite map shall contain a north orientation , loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment, If a site map element is not applicable for your facility then list it on the map and label as "NA", CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #7 - Failed to electronically submit an adequate ER procedures for a release or threatened release.
