The following is an itemized list of hazardous materials violations that have not been <br /> addressed for BLUE SHIELD OF CALIFORNIA as of September19, 2022, <br /> Open violations from March 23, 2022 inspection <br /> Violation #6 - Failed to electronically submit a site map with all required content. <br /> OBSERVATION The site map is not complete. The site map does not show/address the following required <br /> elements: <br /> 1 , Internal roads, <br /> 2, Storm and sewer drains, <br /> 3. The main access and exit points to the structure are not shown, <br /> 4. Emergency response equipment (spill control). <br /> 5, Evacuation staging area. <br /> 6, Emergency shutoffs. <br /> 7, The locations of the hand sanitizer. <br /> 8. Much of the information on the site map is not legible, <br /> REGULATION GUIDANCE: Asite map shall contain a north orientation , loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment, <br /> If a site map element is not applicable for your facility then list it on the map and label as "NA", <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 - Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION The emergency response(ER) procedures were lacking or inadequate. The phone number for <br /> CUPA/UPA, the Environmental Health Department (EHD), 209468-3420 was not identified. <br /> REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including , but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers,calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval, <br /> Page 2 of 5 <br />
