The correct dates for <br /> Secondary Containment Testing is 9/25/2020 and for the Overfill Prevention is 5/27/2021.THe monthly reports list <br /> the Secondary Containment Testing date as 10/15/2020 and the Overfill Prevention as 4/11/2019. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. <br /> OBSERVATION: The Facility Employee Training Certificate was not found on site. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 5/28/2022. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
