I Option 2: Request an Appeal by filing a Statement of Issues. EHD agrees to settle this matter as set forth in the attached Stipulated Penalty Order (Attachment 2). Through this settlement offer, the EHD has chosen to accept a penalty in an amount less than the amount prescribed in the Penalty Order (Attachment 1) If you choose this option, you must complete the following within 15 business days of receipt of this notice: 1. Sign and return the attached Stipulated Penalty Order (Attachment 2); 2. Immediately submit to the EHD, for review and approval, a corrective action plan detailing how the site will be brought into compliance. 3. Perform required compliance activities by compliance deadlines listed in the Stipulated Penalty Order (Attachment 2). Choosing this option and completing the above requirements will resolve the violations and penalties cited in the Penalty Order. Option 2 You may appeal the stated violations by completing the following: Request A 1.
