I <br /> Option 2: Request an Appeal by filing a Statement of Issues. <br /> EHD agrees to settle this matter as set forth in the attached Stipulated Penalty Order <br /> (Attachment 2). Through this settlement offer, the EHD has chosen to accept a <br /> penalty in an amount less than the amount prescribed in the Penalty Order <br /> (Attachment 1) <br /> If you choose this option, you must complete the following within 15 business days of <br /> receipt of this notice: <br /> 1. Sign and return the attached Stipulated Penalty Order (Attachment 2); <br /> 2. Immediately submit to the EHD, for review and approval, a corrective <br /> action plan detailing how the site will be brought into compliance. <br /> 3. Perform required compliance activities by compliance deadlines listed in the <br /> Stipulated Penalty Order (Attachment 2). <br /> Choosing this option and completing the above requirements will resolve the violations <br /> and penalties cited in the Penalty Order. <br /> Option 2 You may appeal the stated violations by completing the following: <br /> Request A 1. Submit a written Statement of Issues, providing the reason(s) why you should <br /> Hearing not be subject to the imposed penalties postmarked within 15 business days of <br /> receipt of this notice. <br /> 2.
