Item #712 Violation code: 112.8(c)(2)Failed to provide and maintain adequate secondary <br /> containment for bulk storage tanks. <br /> OBSERVATION: Facilityfailed to ensure that tanks are inspected and tested by an <br /> appropriately qualified person in accordance with industry standards. The following <br /> observations were noted: <br /> -Monthly and annual inspections are not being conducted for the 600 gallon and 241-gallon <br /> balers <br /> -Monthly inspections for the 500-gallon oil tanks and 1000 gallon used oil tank are being done <br /> on the portable container inspection report instead of the tank monthly inspection report <br /> Response: CVWS conducted Monthly SPCC Inspections on Balers on August 23,2021 and <br /> September 08,2021 and instructed site personnel performing Monthly SPCC inspections on <br /> 500and 1000-gallons tanks to ensure correct information is placed on proper inspection form. <br /> Additional SPCC training will be conducted to ensure inspections forms are filled out correctly. <br /> Please see inspection forms Attachment D. <br /> Item #719 Violation code: CFR 112.8(c)(11) Failed to locate properly or provide sufficient <br /> secondary containment for mobile/portable containers. <br /> OBSERVATION: The facilityfailed to furnish a secondary means of containment, such as a dike <br /> or catchment basin, sufficient to contain the capacity of the largest single compartment or <br /> container with sufcient freeboard to contain precipitation. The following portable containers <br /> were observed with insufficient secondary containment: <br /> A total of eight 55-gallon drums of oil were observed in the secondary shop on four different 30- <br /> gallon Eagle model 1632 spill pallets (2 drums per spill pallet) <br /> -Two 55-gallon drums of oil were observed in the primary shop on two 30-gallon Eagle model <br /> 1632 spill pallets (one drum per spill pallet) <br /> -Two 55-gallon drums of oil were observed in the primary shop on a 30-gallon Eagle model <br /> 1632 spill pallet <br /> -One 70-gallon portable diesel tank was observed in the maintenance wash bay on a 30-gallon <br /> Eagle model 1632 spill pallet <br /> Response: On September 09, 2021 CVWS replaced 30-gallon spill pallets noted in this violation <br /> with 66 and 132- gallon spill pallets.A total of eight 55-gallon drums of oil in the secondary <br /> shop are now sitting on four 66-gallon spill pallets, A total of four 55-gallon drums of oil in <br /> primary shop are now sitting on two 66-gallon spill pallets and the 70-gallon portable Diesel <br /> tank in wash bay is now sitting on al 32-gallon spill pallet. Please see pictures Attachment E. <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Item #114 Violation code: CCR 66262.40(a) Failed to keep signed copy of manifests from <br /> the designated facility for three years. <br /> OBSERVATION: Designated facility signed, and dated copies of Uniform Hazardous Waste <br /> Manifests (UHWM) were not found on-site.
