Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />April 05, 2022 <br /> Facility Address: <br /> 1904 DANIELS ST, MANTECA <br /> Facility Name: <br /> ARCO AM/PM #83831* <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for carbon dioxide, diesel and gaoline were not <br />complete. The forms did not have storage pressure or storage temperature information. <br />REGULATION GUIDANCE: The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: The information was updated in CERS during the inspection. This was corrected on site. <br />This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The following elements were missing from the map: <br />- fuel loading area <br />- carbon dioxide location <br />- propane location <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: The map was updated and uploaded into CERS during the inspection. This was corrected <br />on site. <br />This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. <br />- The 2022 business plan submittal was submitted late on 4/4/2022. <br />- The 2021 business plan submittal was submitted late on 9/2/2021. <br />- The 2020 business plan submittal was submitted late on 4/6/2020 and 6/26/2020 and was incomplete, only facility <br />information and hazardous materials inventory sections were submitted. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Ensure business plan submittals are made annually by January 15 by logging into the <br />CERS at http://cers.calepa.ca.gov/, entering the correct or updated information, and submitting to the EHD for <br />approval. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />FA0024059 PR0542991 SC001 04/05/2022 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 3 <br />1868 E.
