The following is an itemized list of hazardous materials violations that have not been <br /> addressed for BOKISCH INC. as of August 10, 2022. <br /> Open violations from March 29, 2022 inspection <br /> Violation #5 - Failed to electronically submit hazardous materials inventory for all reportable materials on <br /> site. <br /> OBSERVATION At least 842 cubic feet of compressed oxygen, 371 cubic feet of acetylene, two 55 gallon drums of <br /> pump oil, two 55 gallon drums of duratran oil, one 55 gallon drum of 15-40 motor oil, one 55 gallon drum of power <br /> THF (hydraulic fluid), one 55 gallon drum of waste coolant, a 1 , 000 gallon tank of diesel, a 500 gallon tank of <br /> gasoline, at least 10,200 pounds of sulfur, 1 ,210 pounds of calcium hypochlorite, and eight 30 pound containers of <br /> diphacinone were found on site that have not been reported to CERS. <br /> REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #6 - Failed to electronically submit a site map with all required content. <br /> OBSERVATION A site map was not completed and submitted. <br /> REGULATION GUIDANCE: Asite map shall contain a north orientation , loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as "NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 1 <br />
