I also changed the Oxygen &Acetylene to be above
ambient.
I hope I have covered everything. I have re-submitted to CERS but I wanted to let you know also since they don't.
Thank you for your help.
Jewwi fer Dnvl,i6
Business Manager
Bokisch, Inc.
PO Box 335 Victor, CA 95253
(209) 334-4338 Office
(209) 747-1438 Cell
Blip
From: Robert Lopez [EHD] <rlopez@sjgov.org>
Sent: Monday, November 7, 2022 1:24 PM
To:Jennifer Daniel <Jennifer@bokisch.wine>
Subject: RE: PR0547590; Bokish Inc. Hazardous Materials Program Compliance Review
Jennifer Daniel,
I have reviewed the CERS submittal from 10/26/22 and compliance status and have found the following issues:
• Violation #5—the hazardous materials inventory is not complete/up to date.
1. Oxygen, compressed—the storage pressure was wrongly reported as ambient vs. above ambient.
2. Acetylene—the storage pressure was wrongly reported as ambient vs. above ambient.
3. The chemical location must be identified for each chemical reported in CERS. It is not sufficient to
report the location as "see site map".
• Violation #6,the site map does not show or address the following:
1. The handling/storage area for the diphenadione.
➢ A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer
drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and
1
