Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />April 13,2022 <br />Facility Address: <br />5648 N EL DORADO ST, STOCKTON <br />Facility Name: <br />ST MARYS HIGH SCHOOL <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />RemarksItem # <br />6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is not complete. The site map does not show/address the following required <br />elements: <br />1.Loading areas. <br />2.Storm and sewer drains. <br />3.Access and exit points for structures and gates (including structures inside the shop). <br />4.Evacuation staging area. <br />5.Emergency response equipment (fire extinguishers and spill control). <br />6.Emergency shutoffs. <br />7.The locations of calcium hypochlorite, hydrochloric acid, and sodium bicarbonate. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />This is a repeat violation, Class II. <br />7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />OBSERVATION: The emergency response(ER) procedures were lacking or inadequate. Procedures for the <br />immediate notification of local emergency personnel and the Environmental Health Department (EHD) were not <br />accurate. The phone number for the EHD/CUPA was wrongly identified as 209-953-6212 vs. 209-468-3420. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following : <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br />FA0002168 PR0530852 SC001 04/13/2022 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
