The most recent permit on site <br /> was dated 2021. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the [CURRENT YEAR] permit and retain it on site. <br /> This is a minor violation. <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br /> OBSERVATION: The float and chain in UDC 1/2(southeast)failed to stop the flow of product at the dispenser when <br /> tested. <br /> REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. <br /> CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 9, 2022. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152). To schedule an appointment, please call (209)468-3420. <br /> Note: In the facility's CERS submittal, the Statement of Understanding and Compliance was incorrectly <br /> submitted under the Owner/Operator Written Agreement section. It should be placed in the Statement of <br /> Designated Operator or Miscellaneous sections. The Owner/Operator Written Agreement section contains an <br /> incorrect document.
