The site map does not show/address the following required <br /> elements: <br /> 1. Loading areas. <br /> 2. Internal roads. <br /> 3. Storm and sewer drains. <br /> 4. Access and exit points from the street. <br /> 5. Emergency shutoffs. <br /> 6. The locations of Superclean 4837, trihydroxytriethylamine, isocyanate Part A, polyether polyol Part B, auromatic <br /> amino polyol, and methylene chloride. <br /> 7. The map contains codes that are not defined. <br /> REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as "NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER) procedures were lacking or inadequate. 1) Procedures for the <br /> immediate notification of local emergency personnel and the Environmental Health Department (EHD)were not <br /> accurate. The phone number for the EHD/CUPA was wrongly identified as 209-468-3402 (a nonworking phone <br /> number)vs. 209-468-3420. 2) Evacuation plans and procedures, including immediate notice, for the site were not <br /> complete. The Emergency Assembly Area has not been identified. <br /> REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> FA0010531 PR0520402 SCO01 04/19/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 3 of 5 Hazardous Materials Program OIR <br /> 1868 E.
