A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DBA CIRCLEK, REFUEL PETROLEUM INC. 419 S MAIN ST, MANTECA May 23, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 117 CCR 2637(e), 2637(f)Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. <br /> OBSERVATION: Secondary containment testing was performed on 05/05/2021 and a test report was submitted on <br /> 05/04/2022,ten months late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a minor violation. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -The 2021 Monitoring system certification and spill container test results were not on site during the inspection <br /> -The 5/05/2021 Secondary containment test results were not on site during the the inspection. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 208 CCR 2716(a-e)Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -The DO reports did not include the secondary containment testing date. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> FA0003685 PR0231433 SCO01 05/23/2022 <br /> EHD 23-01 Rev.12/06/2021 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
