OBSERVATION: The Hazardous Materials Inventory forms for oxygen, acetylene, and propane were not complete. The max daily amounts in CERS are incorrect. The max daily amount for propane should be listed at 499 gallons not at 150 gallons.The max daily amount for acetylene should be listed at 412 cubic feet not at 180 cubic feet..The max daily amount for oxygen should be listed at 846 cubic feet not at 200 cubic feet. REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part of the business plan. CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a minor violation. 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. OBSERVATION: The site map is incomplete. The site map is missing evacuation staging areas. REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a minor violation. 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. OBSERVATION: The emergency response(ER)procedures were lacking or inadequate. The emergency response and contingency plan is missing the location of th evacuation staging areas and name and phone number of the nearest medical facility. REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to,the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice,for the site CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
