RAM IREZ <br />Report emailed to syedr@mcdgs.org and 24092@mcdgs.org <br />03/22/2022 I received an email from Program Coordinator saying that this compliant is similar <br />to a previous complaint 1/25/2022. <br />RAM IREZ <br />This complaint is chargeable. <br />I emailed report and explained to manager the charge syedr@mcdgs.org. <br />and 24092@mcdgs.org. <br />I also called the facility and corporate office and left voice mail messages there <br />was no answer. <br />I received a call later in the day while conducting an inspection from Jeffrey Liu, <br />the manager of the facility. <br />I discussed the inspection with him. <br />Jeff Liu sent me an email saying that the facility will follow up with staff at the <br />facility. <br />5105.rpt <br />Page 1 of 1
