<br /> <br /> <br /> <br /> <br />Robert Lopez May 23, 2022 <br />Hazardous Materials Specialist II <br />San Joaquin County Environmental Health Department <br />1868 E Hazelton Avenue <br />Stockton, CA 95205 <br /> <br />Dear Mr. Lopez, <br />This letter is provided in response to the inspection report and email received from you dated April 28, 2022. <br />Attached to this letter, you will find documentation responsive to the violations noted during your April 28 <br />inspection of the Former Kearney-KPF Facility, located at 1624 E Alpine Avenue, Stockton, CA 95205. The <br />violations, and our responsive materials (in italics), are summarized below. <br /> Violation of HSC 25505(a)(2): Failed to electronically submit a site map with all required content. <br />A revised site map, which was prepared in accordance with the Hazardous Materials Site Map template <br />provided by San Joaquin County Environmental Health Department, was uploaded to CERS on May 23, <br />2022. <br /> Violation of HSC 25505(a)(3): Failed to electronically submit adequate ER procedures for a release or <br />threatened release. <br />A revised Emergency Response/Contingency Plan, which was been prepared in accordance with the <br />CERS Consolidated Emergency Response/Contingency Plan (ER Plan) template provided by San Joaquin <br />County Environmental Health Department, was uploaded to CERS on May 23, 2022. <br /> Violation of HSC 25505(a)(4): Failed to establish and electronically submit a reasonable or an adequate <br />training program. <br />Dudek personnel who manage the treatment system at the former Kearny-KPF Facility have received <br />appropriate training. An Employee Training Record Form, which was prepared in accordance with the <br />example Employee Training Sign-In Sheet provided by San Joaquin County Environmental Health <br />Department, was uploaded to CERS on May 23, 2022, attached to the Emergency <br />Response/Contingency Plan. <br /> Violation of HSC 25505(a)(4): Failed to provide or document initial and/or refresher training to <br />appropriate personnel. <br />Dudek personnel who manage the treatment system at the former Kearny-KPF Facility have received <br />appropriate training. The Employee Training Record Form was uploaded to CERS on May 23, 2022, <br />attached to the Emergency Response/Contingency Plan. <br />
