Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />May 06, 2022 <br /> Facility Address: <br /> 2140 Daniels St, Manteca <br /> Facility Name: <br /> Five Guys Manteca <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br />OBSERVATION: The Business Owner/Operator Identification form was not complete. Scott Brander is listed as <br />primary emergency contact and environmental contact but has not been employed by business in over three years. <br />Update the primary emergency contact and environmental contact information. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for Carbon dioxide were not complete. Obsolete federal <br />hazard categories were submitted on the chemical fact sheet. Use an updated Safety Data Sheet (SDS) to select <br />the correct federal hazard classes. <br />REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The site map is missing storm drain locations, adjacent streets, a <br />north orientation, loading area, emergency and utilities shut off (water, power, natural gas, carbon dioxide). <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br />FA0023728 PR0541410 SC001 05/06/2022 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
