Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />May 06, 2022 <br /> Facility Address: <br /> 2140 Daniels St, Manteca <br /> Facility Name: <br /> Five Guys Manteca <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. The business plan was not <br />submitted for the years of 2018, 2019, 2020, 2021, and 2022. The last submittal made was on 12/6/2016 for the <br />year of 2017. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br />information, and submit to the EHD for approval. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Conducted in-office Hazardous Materials Business Plans (HMBP) inspection. The food inspector, Gehane <br />Fahmy, performed an on-site inspection for the CO2 tank on 7/23/2021. <br />There were 6 violations found during the in-office inspection. complete and submit a copy of the Return to <br />Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or <br />will be taken for each violation, and any supporting paperwork, by 6/5/2022. <br />Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
