The following is an itemized list of underground storage tank violations that have not been addressed for MANTECA GAS & FOOD* as of October 25, 2022. Open violations from August 16, 2022 inspection Violation#115-Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. OBSERVATION:Annual monitoring system certification, leak detector testing, and spill container testing was performed on 8/12/2021 and a test report was submitted on 9/15/2021,4 days late. REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#116-Failure to submit the"Spill Container Testing Report Form"within 30 days. OBSERVATION: Maintenance and monitoring records for the last three years were not found on site.The work order for the replacement of the 87, 91, and diesel drop tubes was not available for review. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. Page 1 of 4
