The following is an itemized list of underground storage tank violations that have not been addressed for MARCH AND BIANCHI INC as of May 10, 2022. Open violations from November 30, 2021 inspection Violation#304-Failure to construct,operate,and maintain primary containment as product-tight. OBSERVATION: Little amounts of oil was observed in under dispenser containment sump (UDCs)7/8, 9/10 and 11/12 indicating a leak in the primary containment. REGULATION GUIDANCE:All primary containment for the UST system must be product tight. CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the failed component(s), under permit and inspection of the EHD if necessary. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#315-Failure to maintain interstitial space to detect leaks before release to environment(post-Jul 2004). OBSERVATION: The diesel smart sensor failed when tested. REGULATION GUIDANCE: The interstitial space of the UST shall be maintained, as designed, under constant vacuum or pressure such that a breach in the primary or secondary containment is detected before the hazardous substance is released into the environment. CORRECTIVE ACTION: Correct immediately by having a properly licensed,trained, and certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable), and obtain a permit within one business day from the EHD, if needed.
