Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />May 09, 2022 <br /> Facility Address: <br /> 6220 E HWY 12 , LODI <br /> Facility Name: <br /> PLASTAKET MFG INC <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br />OBSERVATION: The Business Owner/Operator Identification form was not submitted. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: This violation was corrected at the time of the inspection. <br />This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: 60 gallons of MasterSeal HLM 5000 were found on site that have not been reported to CERS. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: This violation was corrected at the time of the inspection. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: A site map was not completed and submitted as part of the business plan. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: This violation was corrected at the time of the inspection. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />OBSERVATION: The emergency response(ER) procedures have not been submitted in CERS. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: This violation was corrected at the time of the inspection. <br />This is a Class II violation. <br />FA0009273 PR0519509 SC001 05/09/2022 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 3 of 6 <br />1868 E.
