A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> TIGER EXPRESS STORES 1399 E YOSEMITE AVE, MANTECA June 02, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 202 CCR 2712(i) Failure to retain a paper or electronic copy of the permit to operate at the facility. <br /> OBSERVATION:A copy of the UST operating permit was not found on site. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the permit and retain it on site. <br /> This is a repeat violation, Class Il. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -The 2019 Monitoring system certification test results were not found on site during the inspection. <br /> -The 07/2/2019 Secondary containment test results were not found on site during the inspection. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class Il. <br /> 214 CCR 2712 Failure to comply with UST operating permit conditions. <br /> OBSERVATION: The diesel drop tube was replaced on 06/22/2021 and a permit was not obtained from the EHD. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the replacement of the diesel drop tube .
