Your notification is administratively complete, but has not been reviewed for technical <br />adequacy. A technical review of your notification will be conducted when an inspection is performed. You <br />may be subject to penalty if violations of either laws or regulations are found. <br />Pursuant to Title 22, California Code of Regulations (CCR), Section 67270.60(d)(5), you are hereby <br />authorized to operate under permit by rule the temporary household hazardous waste collection facility identified <br />above on the date(s) specified or alternate date, if necessary. <br />You must renotify the Department whenever any of the information provided in your initial notification <br />changes significantly. To renotify, mail a cover letter to the above address explaining the change(s), along with <br />a copy of the revised Form 8464. The revised Form 8464 should have an asterisk in the left margin next to the <br />revised information; it should also be re-signed and re -dated at the signature space on page 4. <br />As set forth in Title 22, CCR, section 67450.9(c), this authorization is contingent upon the accuracy of <br />the information submitted in the notification mentioned above. Any misrepresentation or any failure to fully <br />disclose all relevant facts shall render this authorization to operate null and void. <br />Pursuant to Health & Safety Code Section 252.05.1, household hazardous waste collection facilities <br />operated by local government agencies are exempt from all fees imposed by Article 9.1 of Health & Safety <br />Code.
