Upon obtaining the <br />appropriate signature, retain a copy for your records and mail the original to The San Joaquin <br />County Environmental Health CUPA Office via certified mail -return receipt required Upon <br />receiving the receipt from San Joaquin County Environmental Health, please facsimile to MSE a <br />copy of the signed PBR(s) and the returned certified receipt. We will incorporate these into the <br />operation plans. Please forward copies of all signed PBR(s) to DTSC at the address below. <br />We look forward to smooth running events.
