Code Regs., Chapter 20 for <br />permitted facilities, except as specified in section IV. <br />IV. TERMS AND CONDITIONS <br />Conditions for revocation: This variance is subject to review at the discretion of <br />DTSC and may be revoked at any time. DTSC shall revoke this variance if DTSC finds <br />that the provisions as outlined in HSC, section 25143(c) for variance revocation have <br />been met or on the basis of changes in DTSC policy. <br />Specific terms and conditions of this variance: This variance is issued on the basis <br />of information contained in the variance application submitted to DTSC on May 12, <br />2000. The applicant shall be subject to the following conditions: <br />1. This variance applies only to the hazardous waste collection site specified in <br />section I. of this document. If there are any changes in the location or date of the <br />scheduled event, the applicant shall notify DTSC at the address identified in <br />"Correspondence" below in writing at least ten (10) days prior to the rescheduled event. <br />2. The HHW collection shall be conducted in accordance with the information and <br />requirements set forth in this variance, the application operation plans, and all <br />applicable State, Federal and local regulations. Failure to comply may result in denial <br />for future operations or lead to enforcement actions. <br />3. All collection operations shall be conducted to minimize potential harm to the <br />public, operators, haulers, and the environment. All associated collection personnel, <br />contractors, and emergency response personnel who will be handling hazardous waste <br />shall use all required personal protective and safety equipment during operating hours <br />as specified by Title 8, Cal. Code Regs. <br />4. Traffic control at the collection site shall be monitored by applicant or contractor <br />staff. Collection personnel shall be responsible for removing the hazardous waste from <br />the participant's vehicle. Waste shall be immediately taken to a staging area by trained <br />personnel. Only trained personnel shall be allowed to enter any waste handling areas. <br />5. Only those persons trained in hazardous waste management including personnel <br />unloading waste from vehicles shall be allowed to handle the waste. Job titles, job <br />descriptions, and personnel training records shall be made available on-site for each <br />person handling hazardous waste as specified in Title 22, Cal. Code Regs., section <br />66264.16(d) to substantiate training. <br />
