6. Give the proper DOT shipping description for the HW, if applicable, tList standard DOT classes, if known
<br />The program accepts miscellaneous industrial and household hazardous waste which includes: paint, solvents,
<br />cleaners, aerosols, asbestos, photo chemicals, used oil, oil filters and antifreeze. There is not one DOT shipping
<br />description to fit all wastes being collected, however, the following are the shipping descriptions of the most commonly
<br />collected wastes:

<br />7. Give the name of registered transporter or hauler?
<br />The CESQG participants or their employees will transport hazardous waste generated by the participants and only in
<br />vehicles owned and operated by participants.
<br />8. Where will the HW be taken? Is this an authorized or permitted facility? If stored for consolidation
<br />purposes, give the temporary storage facility address and indicate how long it will be stored at this
<br />location.
<br />The hazardous waste will be transported to one of the temporary HHW event sites operated by the County of San
<br />Joaquin. A proposed schedule of events and locations is attached. Please note that all EPA ID numbers included on
<br />the list are pursuant to Permit By Rule Notification for Temporary Household Hazardous Waste Collection Locations.
<br />At the collection location, the CESQG waste will be consolidated and packaged for transportation by the contractor,
<br />MSE Environmental, Inc. The CESQG waste will be shipped from the collection location within 144 hours. All CESQG
<br />waste will be ultimately transported to permitted recycling or TSD facilities.
<br />9. Do you have the proper insurance coverage for transporting hazardous waste? If not and you wish a
<br />variance from the insurance requirements, give your reasons why you believe the insurance coverage is
<br />not necessary. Tell us what types and amounts of insurance coverage you and/or your contractor do
<br />carry.
<br />No. CESQGs generally do not have the proper insurance for the transportation of hazardous waste. We are
<br />requesting a variance from these insurance requirements as each participant will be transporting waste in their own
<br />vehicles and Section 25169 of the H&S Code applies only to registered hazardous waste haulers.
<br />10. Will the waste be shipped using a Uniform Hazardous Waste Manifest? If a manifest variance is also
<br />requested, give reasons showing why you need this variance. If a manifest variance is granted, a
<br />shipping paper will be required to accompany the shipment — if so, attach an example of shipping papers
<br />you plan to use.
<br />A manifest will not be used to ship the above described hazardous waste from the CESQG's business to the San
<br />Joaquin County Temporary Collection site. The generator will be required to have a copy in their vehicle of the
<br />man ifest/transportation variance (being applied for with this form) authorizing them to transport up to 27 gallons or 220
<br />pounds of hazardous waste to the facility. The generator will also be required to have the CESQG Certification and
<br />Inventory Form (see attached) with them to serve as their shipping paper. Once the wastes is received at the
<br />collection location, MSE Environmental, Inc. will properly package the waste prior to transportation. The waste will
<br />then be manifested and transported to a permitted TSDF.
<br />11. If bulking any wastes (e.g., oil, latex paint, solvents, flammables, etc.) provide the contact name, phone
<br />number, and mailing address of your local fire department and the local Air Quality Management District.
<br />CESQGs will not be bulking their waste prior to transporting.
