COMPLAINT INSPECTION REPORT <br />11:30 am <br />11:00 am <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: VIP PIZZA Date: 05/10/2022 <br />Address: 3499 BROOKSIDE RD, STOCKTON 95219 <br />Owner/Operator: HUGHES, JOHN M Telephone: (209) 957-9754 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0054841 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT VISITED FACILITY ON 4/22/22 AROUND 4:00 IN THE AFTERNOON. COMPLAINANT ALLEGES THE <br />FLOOR LOOKED AS THOUGH IT HADN'T BEEN SWEPT OR MOPPED IN WEEKS AND THE ESTABLISHMENT OVER <br />ALL WAS DIRTY. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />No Temperature Data Collected <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />COMMENTS AND OBSERVATIONS <br />On site to conduct complaint investigation. Per Trustee, he was unaware of the complaint prior to this investigation. Did not <br />observe floors that looked as though they have not been swept or mopped in weeks. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
