  <br />21  <br />        <br />TABLE 2  <br />EMERGENCY INFORMATION (continued)  <br />  <br />DIRECTIONS TO EMERGENCY HOSPITAL (see Figure 2):  <br />  <br />From Site: Head north on Cambridge Dr toward E Louise Ave for 125 ft. Turn right at the 1st cross  <br />street onto E Louise Ave and continue on for 0.7 miles, then turn right onto McKinley Ave.  <br />Continue on McKinley Ave for 1 mile, then turn left onto W Yosemite Ave. Continue on W  <br />Yosemite Ave for 1.4 miles, then turn left. The destination will be on the right and the emergency  <br />room entrance will be well marked.  <br />  <br />Kaiser Permanente Manteca Medical Center  <br />1777 W Yosemite Avenue   <br />Manteca, California  <br />(209) 825‐3700  <br />  <br />STANDARD PROCEDURE FOR REPORTING EMERGENCIES  <br />  <br />When calling for assistance in an emergency situation, provide the following:  <br />  <br />• Name of person making call.  <br />• Telephone number and location of person making call.  <br />• Name(s) of person (s) exposed or injured and their location.  <br />• Nature of emergency and type of exposure, when appropriate.  <br />• Actions already taken.  <br />Never be the first to hang up when calling for emergency assistance.  <br />  <br />Wait for the dispatch operator to finish all questions.  <br />  <br />As with all wireless phones, if you are in an area where your phone is searching or scanning for a  <br />signal, it is highly probable that a call to 911 will not go through.  Prior to beginning site activities,  <br />the nearest landline phone will be identified.  If your wireless 911 call is not connecting, use the  <br />nearest landline phone and call for help or call the fire department directly.  <br />
