Failing to electronically submit a site map showing loading areas, internal roads, <br />storm drains, sewer drains, emergency response equipment, evacuation and staging <br />area, access and exit from the street, emergency shutoff, the location of antifreeze and <br />waste antifreeze, in violation of Health and Safety Code section 25505(a)(2); <br />m. Failing to electronically submit emergency response procedures for a release or <br />threatened release of hazardous waste material, in violation of Health and Safety Code <br />section 25505(a)(3); <br />n. Failing to establish and electronically submit an employee training prograrYi regarding <br />procedures for the mitigation of a release or threatened release of hazardous material, <br />evacuation plans, and notification to Local emergency response personnel, in violation <br />of Health and Safety Code section 25505(a)(4); <br />o. Failing to provide an emergency response plan and site map to personnel o£ the <br />business or personnel of the San Joaquin County Environmental Health Department, <br />in violation of Health and Safety Cade section 2SSOS(c); <br />p. Failing to annually certify and submit a Hazardous Materials Busixiess Plan in <br />the California Environmental Reporting System, in violation of Health and Safely <br />Code section 25508.2. <br />6. Defendant SAO SIN is responsible for the uniform fee for Fling the first paper in a civil <br />action or proceeding in the Superior Court. The amount of the filing fee is FOUR HUNDRED <br />THIRTY-FIVE DOLLARS ($435.00). Said payment shall be made,payable to the SAN JOAQUIN <br />COUNTY SUPERIOR COURT and delivered to the San Joaquin County Office of the District <br />Attorney, Consumer Protection Unit, at the time of si iim�. <br />7. Defendant SAO SIN shall pay to Plaintiff the sum of FOUR HUNDRED EIGHTY- <br />THREE DOLLARS ($483.00) as partial recovery of the cost of investigation and suit incurred by the <br />SAN JOAQUlN COUNTY ENVIORNMENTAL HEALTH DEPARTMENT.
