OBSERVATION: Secondary containment testing was performed on 11/26/21 and a test report was submitted on 3/22/22, 86 days late. REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a repeat violation, Class II. 118 CCR 2637.2(d&e), 2665(a&b) Failure to submit the"Overfill Prevention Equipment Inspection Report Form"within 30 days. OBSERVATION: Overfill prevention equipment was performed on 10/11/21 and a test report was submitted on 11/22/21, 12 days late. REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a repeat violation, Class II. 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The missing records include: -2020 monitoring system certification test results -2021 monitoring system certification test results -2021 secondary containment test results -Work order from the 91 leak detector replacement on 12/21/21 REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site. Submit copies of the missing work orders to the EHD.
