Forward the reports to the E H D if on file at <br />Your facility. Backflow reports are due for 2010 by December 31, 2010, <br />2) Provide copy of the CDO license for Jonathan Cook by 3125/2010. <br />3) A J -vent is lacking at the welt -head. The J -vent shalt be tumed down with the opening screened to prevent entry into <br />the well by vectorslinsects and to prevent bacteriological contamination. Install by 3125/2020. This was noted on <br />the last inspection dated 101712008. <br />4) Complete a New Sample Siting Pian (enclosed) highlighted sections only and return via mail or FAX to (209) 468- <br />0341 BY 312 512 0 1 0, <br />5) The Well in the parking lot (Well #1) has been inactive since January of 2006. A letter of commitment dated <br />517/2009 indicated that the plan for the well was to be for °dean and washing customers RV's prior to them picking <br />their RV's up for traveling". Upon inspection this date, it did not appear that RVs' are being stored in the parking lot <br />and that the parking lot is utilized by visitors and customers that are frequenting the newly opened cafe. Provide a <br />new letter of commitment or plan to bring the well back into service under permit and Inspection and/or destroy the <br />well under permit and inspection by 3/3112010. <br />6) Provide AVBs (Hose bib Atmospheric Vacuum Breakers) at restaurant west -side hose bib and any other locations <br />where Rose bibs are currently in use. Provide by 3131 120 1 0. <br />A water meter has been installed that is a totalizer and before distribution.
