Robert Lopez [EHD]
From: Robert Lopez [EHD]
Sent: Thursday,July 21, 2022 7:49 AM
To: Sukh Bal
Subject: RE: PR0541978; Subway Franchise#67296 Materials Program Inspection Report
Attachments: Service Request Form.pdf
Sukhnandan Bal,
The Hazardous Materials Business Plan must be completed and submitted in the California Environmental Reporting
System (CERS) by July 21, 2022;this includes, but is not limited to the submittal of documentation of employee training.
Regretfully, I am not authorized to extend due dates for Hazardous Material Business Plan violations. It is the policy of
the Environmental Health Department to make notifications as a reminder of the upcoming due date then, send past
due notice(s) once the violation due date has passed; all time spent attempting to obtain compliance may be billed at an
hourly rate.
Regarding your request for assistance, you may call me to schedule a date and time for a CERS consultation at the EHD. I
have attached a service request form to be filled out prior to the start of the consultation. You may complete the form
and send it back to me or bring it the day of the consultation.
On the day of the appointment, will need to bring the CERS user name, password, and payment for the first hour of the
consultation ($156), if the event consultation goes past 1 hour, you will be billed for the balance.
As of July 1, 2022, all in-office CERS help will be provided at EHD hourly rate ($156). A one hour fee is required to be
paid before the start of the consultation.
Respectfully,
Robert Lopez
Hazardous Materials Specialist II
San Joaquin County
Environmental Health Department
1868 E.
