The hazardous materials inventory form for the carbon dioxide is not <br /> complete/accurate. <br /> 1. The average daily amount element has not been completed. <br /> 2. The size of the largest container element has not been completed. <br /> 3. The storage pressure is wrongly reported as "ambient" vs. "above <br /> ambient". <br /> 2. Violation#6 has been closed. <br /> 3. Violation#7, Emergency Response/Contingency Plan is not complete. <br /> 1. Section E. the location of the Emergency Assembly Area has not been reported. <br /> 1. Violation#8, employee evacuation training records were uploaded in lieu of an Employee <br /> Training Plan. <br /> - A training plan must address training for all new employees and annual training, <br /> including refresher courses, for all employees in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to: <br /> 1. Familiarity with the emergency response plans and procedures in the event <br /> of a release or threatened release of a hazardous material. <br /> 2. Immediate notification contacts to the appropriate local emergency <br /> response personnel and to the unified program agency. <br /> 4 <br />
