COMPLAINANT ORDERED A PERSONAL SAUSAGE <br />PIZZA AND A SALAD THAT CONTAINED CROUTONS, KIDNEY BEANS, GARBANZO BEANS, AND DRESSING. AT <br />AROUND 3:00 PM, COMPLAINANT BEGAN TO EXPERIENCE STOMACH PAINS, NAUSEA, DIARRHEA, AND A <br />HEADACHE. COMPLAINANT IS STILL FEELING STOMACH PAINS AT THE TIME OF COMPLAINT. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />Diced tomatoes -- three door Atosa prep reach-in cooler -- 38.00° <br />Mozzarella — three door Turbo Air prep cooler —41.00° F <br />Pizza warmer-- 136.00° F <br />Garbanzo/kidney bean mixture -- three door Atosa prep reach-in <br />cooler — 38.00° F <br />Ranch dressing — three door Atosa prep reach-in cooler -- 39.00° <br />Sausage -- three door Turbo Air prep cooler -- 33.00° F <br />COMMENTS AND OBSERVATIONS <br />On site to conduct complaint investigation. Per Elaine Gouskos (Manager), she was unaware of the complaint and did not <br />receive any similar complaints. No illnesses were reported by any employees. Temperatures of sausage, mozzarella cheese, <br />ranch dressing, garbanzo/kidney beans, diced tomatoes and pizza warmer were all within safe temperature ranges. Hot and <br />cold water available at facility. Sanitizer available at facility. Soap and paper towels available at facility. <br />Discussed report with Elaine Gouskos (Manager). Official inspection report was emailed to the operator. <br />To minimize person-to-person contact, the signature of the person receiving the inspection report was not captured. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
