Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />June 23, 2022 <br /> Facility Address: <br /> 23955 S CHRISMAN RD, TRACY <br /> Facility Name: <br /> 11TH ST CHEVRON AUTOMOTIVE SERVICE & <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for used automotive oil was not complete. Used <br />automotive oil did not have largest container size, day on site, storage pressure, and storage temperature. The max <br />daily amount is listed at 5 pound. The max daily amount should be listed at 500 gallons. 55 gallons of used anti <br />freeze and 700 gallons of new oil was not reported on the chemical inventory. <br />REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This violation was corrected on site. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete because it is missing a key or legend identifying what SD and FE <br />mean. Evacuation staging areas, adjacent streets, and loading area were nt identified on the site map. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This violation was corrected on site. <br />This is a repeat violation, Class II. <br />FA0024637 PR0543406 SC001 06/23/2022 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
