The following is an itemized list of hazardous materials violations that have not been <br /> addressed for 7- ELEVEN #38153 as of September 26 , 2022 . <br /> Open violations from April 12, 2022 inspection <br /> Violation #5 - Failed to electronically submit hazardous materials inventory for all reportable materials on <br /> site . <br /> OBSERVATION The Hazardous Materials Inventory forms for Carbon Dioxide were not complete . Carbon Dioxide <br /> was reported on the California Environmental Reporting System ( CERS ) , was reported at 53 liquid gallons . <br /> Observed an approximate 300 liquid gallon CO2 tank. <br /> REGULATION GUIDANCE : The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan . <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System ( CERS ) at <br /> http : //cers. calepa . ca . gov/ , enter the correct or updated information , and submit to the EHD for approval . <br /> ❑ This violation was corrected 11This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel . <br /> OBSERVATION Jesse Singh , Manager, could not provide employee training on ; <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> ( B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property , or the environment <br /> ( C ) Evacuation plans and procedures , including immediate notice , for the business site. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training . All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material , including , but not limited to , familiarity with the following <br /> plans and procedures : <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency . <br /> ( B ) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property, or the environment <br /> ( C ) Evacuation plans and procedures , including immediate notice , for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years . <br /> CORRECTIVE ACTION : Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD . <br /> Page 1 of 2 <br />
