Mountain House 95391 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the application <br /> for a permit to install a new underground storage tank (UST) system at the above noted address, which will <br /> consist of: 30,000 gallon Regular Unleaded <br /> 15,000 gallon Premium Unleaded <br /> 15,000 gallon Diesel <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local Fire District, the San Joaquin Valley Air Pollution Control District, and the local <br /> Community Development Department for applicable permits. <br /> 2. Any changes shall be submitted to the EHD for approval prior to commencing work, including <br /> change in contractors, service technicians, installers, etc. All new submittals supersede the <br /> previous ones. <br /> 3. Qualified installers must possess or work under the direct and personal supervision of an individual <br /> physically present at the work site, at all times while work is being performed, who possesses all of the <br /> following: <br /> • One of several permissible valid, current contractor's licenses issued by the Contractors State <br /> License Board (may be issued to the installation company) <br /> • A valid, current UST Installation/Retrofitting certificate issued by the International Code Council <br /> (ICC) <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) being installed <br /> 4. Qualified service technicians must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site, at all times while work is being performed, who possesses <br /> all of the following: <br /> • One of several permissible valid, current contractor licenses issued by the Contractors State <br /> License Board (may be issued to the installation company) <br /> • A valid, current California UST Service Technician certificate issued by the International Code <br /> Council (ICC) <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) being tested, <br /> repaired, or serviced <br /> 1868 E.
