The following is an itemized list of hazardous materials violations that have not been <br /> addressed for CHIPOTLE MEXICAN GRILL#2865 as of December 05, 2022. <br /> Open violations from July 11, 2022 inspection <br /> Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Employee training records did not cover the initial and annual refresher training.All employees <br /> shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, <br /> including, but not limited to,familiarity with the following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> A training needs to be conducted where employees review the HMBP and then then complete the sign in sheet and <br /> email it to Nick Loehrer of Environmental Health. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#12-Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Facility could not provide the Emergency Response Plan and the inventory information upon <br /> request. Employees do not have access to the Emergency Response Plan and the inventory information on-site.A <br /> hard copy or electronic copy must be made readily available for employees and for the Environmental Health <br /> Inspector. <br /> REGULATION GUIDANCE: The emergency response plans and procedures,the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> Page 1 of 2 <br />
