REGULATION GUIDANCE: A new form effective 10/1/2020 should be uploaded into CERS when uploading new forms.The owner or operator shall submit a"Designated Underground Storage Tank Operator Identification Form" no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility on the most current 2020 form. OBSERVATION: The Underground Storage Tank Statement of Understanding and Compliance Form was not found in CERS. REGULATION GUIDANCE: This form must be uploaded to the California Environmental Reporting System (CERS). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload the Underground Storage Tank Statement of Understanding and Compliance Form for this facility. This is a Class II violation. 115 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. OBSERVATION:Annual monitoring system certification, leak detector testing, and spill container testing was performed on 7/6/2021 and a test report was not submitted to EHD.A hard copy was provided to EHD during the inspection, 347 days late. REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a repeat violation, Class II.
