OBSERVATION: Designated operator reports were not signed within the required time frame. -Multiple designated operator reports were not signed by the owner/operator including: 6/29/2022, 1/6/2022, 10/7/2021, 9/10/2021, 8/3/2021, 7/8/2021. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. 2. OBSERVATION:The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 5/24/2022, next inspection performed on 6/29/2022(36 days) - Initial inspection on 8/3/2021, next inspection performed on 9/10/2021 (38 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 4/6/2022: -was missing a CERS ID number -lists an initial training date(second to the last column)later than the date of assuming responsibility as a facility employee(last column) REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class II.
