The primary emergency <br />contact is listed as Miguel Pablo. The secondary contact is listed as Wendy Hassall. Adam Monarres who is the <br />current general Manager for the business stated that "Miguel and Wendy have not work for the business in years". <br />Adam Monarre should be listed as primary emergency contact. Marck Farinas should be listed as the secondary <br />emergency contact. The environmental contact also needs to be updated with some one who works for the <br />business. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for Carbon dioxide were not complete. The carbon <br />dioxide chemical fact form has obsolete federal hazard categories listed. Uncheck the obsolete federal hazard <br />categories and the new ones using an updated safety data sheet. The current unit of measure say 500 cubic feet <br />but should say 500 pounds. Update the unit of measure to be reported in pounds. <br />REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The site map is missing loading areas and storm drain locations. A <br />key or legend need to be provide when acronyms and symbols are used.
