OBSERVATION:A permit to operate the UST system has not been issued. Permit to operate was not issued by the EHD due to Class 1 violation remains open from a UST inspection conducted on 8/12/2021. REGULATION GUIDANCE: No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system. CORRECTIVE ACTION: Immediately obtain a permit to operate a UST system from the EHD. This is a Class I violation. 202 CCR 2712(i) Failure to retain a paper or electronic copy of the permit to operate at the facility. OBSERVATION:A current copy of the UST operating permit was not found on site. The most recent permit on site was dated 2022. REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. CORRECTIVE ACTION: Immediately obtain a copy of the 2022 permit and retain it on site. This is a Class II violation. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note the correct test dates for the monitoring certification, and spill container on the designated operator inspection reports. Incorrect test dates for the monitoring certification were observed on the 11/3/21, 12/1/21, 12/30/21. and 1/26/21 DO reports.The date noted was 10/28/2021. Incorrect test dates for spill container were observed on the 2/23/21. 3/23/21.4/20/21. 5/18/21, 6/15/21. 7,/13/21, and 8/11/21. The date noted was 10/28/21.The correct date for monitoring certification is 10/28/21, and spill container testing is 8/12/21. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a Class II violation.
