The following is an itemized list of hazardous materials violations that have not been addressed for BT INDUSTRIAL SERVICES as of September21, 2022. Open violations from July 21, 2022 inspection Violation #4 - Failed to electronically submit the Business Activities or Owner/Operator Identification page. OBSERVATION The Business Owner/Operator Identification form was not accurate. The Business Owner, Primary Emergency Contact, and Environmental Contact, Gary Tofanelli has not been affiliated with this facility since at least July 1 , 2021 . REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calep;a.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #5 - Failed to electronically submit hazardous materials inventory for all reportable materials on site. OBSERVATION At least 60 gallons of adhesive and at least twenty pallets of cement products (greater than 10,000 pounds, actual weight was not available) were found next to the shop that have not been reported to CERS. REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan.
