The site map is missing an evacuation staging area, loading area <br /> (where fuel truck parks), storm drains(if no storm drains put N/A for storm drains on site map), and emergency <br /> response equipment locations(fire extinguishers and spill kits). No site map key or legend on the site map.A site <br /> map key or legends needs to define any symbol or acronyms used. add a sit map key that defines E. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> $ HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br /> OBSERVATION:A training program was not complete. Discard the document in the training program section. <br /> Select that the business training program is provided elsewhere in CERS under the emergency response and <br /> contingency plan section <br /> REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, <br /> shall include familiarity with the facility's Emergency response plans and procedures in the event of a release or <br /> threatened release of a hazardous material.Training shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> These training programs may take into consideration the position of each employee. <br /> CORRECTIVE ACTION: Immediately prepare or update the facility's training program, log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0009437 PR0511725 SCO01 07/28/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
