Norma Gaona is <br /> listed as the primary contact but has not been with the business for years.The current primary contact for the <br /> business is Abraham Ochoa who is the general manager for the business. Update the primary emergency contact <br /> information in the owner operator section. <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The Hazardous Materials Inventory forms for carbon dioxide were not complete.
