S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> JACOBS MALCOLM &BURTT 645 N STOCKTON ST, LODI JUly 20, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 104 CCR 66262.34(d)(2)No modified contingency plan. <br /> OBSERVATION: Modified contingency plan information was not posted at the facility. <br /> REGULATION GUIDANCE: The business operator must post the following information next to telephones or in <br /> areas directly involved in the generation and accumulation of hazardous waste: the name and emergency <br /> telephone number of the emergency coordinator, the location of fire extinguishers and spill control material, and the <br /> telephone number of the fire department unless the facility has a direct alarm. <br /> CORRECTIVE ACTION: Immediately post the modified contingency plan information or keep a copy of a complete <br /> consolidated emergency response/contingency plan on-site. Provide a statement to the San Joaquin County <br /> Environmental Health Department that the correction is complete. <br /> This is a minor violation. <br /> 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: The facility could not provide all of the consolidated manifest receipts for hazardous waste <br /> transported from the site in the past three years. The only Hazardous Waste stream observed at this facility is used <br /> oil. Facility could not locate the manifest receipts for the last three years. <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action regarding <br /> the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Locate a copy of the receipts listed above. Maintain the records on-site for three years <br /> from the date of shipping and provide a copy to the San Joaquin County Environmental Health Department(EHD). <br /> If the receipts cannot be found, provide a statement to the EHD that future receipts will be maintained on-site for <br /> three years. <br /> This is a repeat violation, Class Il. <br /> 601 CCR 66262.34(d)Stored hazardous waste onsite greater than 180 days(90 days acute/extremely). <br /> OBSERVATION: One 55 gallons drum of used oil observed onsite with an accumulation start date of greater than <br /> 180 days . The accumulation start date marked on the label is 2021. <br /> REGULATION GUIDANCE: Title 22 California Code of Regulations section 66262.34. <br /> CORRECTIVE ACTION: Immediately dispose of the hazardous waste stored on site greater than 180 days in <br /> compliance with Title 22 California Code of Regulations.
