The site map does not show/address the following required <br /> elements: <br /> 1. Loading areas. <br /> 2. Internal roads. <br /> 3. Access and exit points from the street. <br /> 4. Emergency response equipment (spill control). <br /> 5. Emergency shutoffs. <br /> 6. Location of the plastisol, colorant, 4543-NATL, and the waste hydraulic oil. <br /> 7. The site map contains undefined codes <br /> REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. If a site map element is not applicable <br /> for your facility then list it on the map and label as "NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER) procedures were lacking or inadequate. The Consolidated <br /> Emergency Response/Contingency Plan was submitted in an unusable format. The submittal did not include: 1. <br /> Procedures for immediate notification of local emergency personnel and the Environmental Health Department <br /> (EHD). 2. Procedures for the mitigation of a release or threatened release to minimize any potential harm or <br /> damage to persons, property, or the environment. 3.
