Time In: 1:10 pm <br /> r <br /> Time Out: 1:30 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: THAI SPICES RESTAURANT Date: 07/26/2022 <br /> Address: 2401 W TURNER RD, LODI 95242 <br /> Owner/Operator: KAMKEM LLC Telephone: (209)369-8424 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00055060 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THE SPRING ROLLS AND FRIED CALAMARI TASTED SOUR. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke to the chef who was not aware of the complaint prior to my investigation. <br /> Spring rolls are made with cabbage, carrots, mushrooms, taro and sauces. <br /> I observed shredded carrots and cabbage that is pre shredded and comes to the facility packaged, stored in the walk-in (38F). <br /> Taro was not available during the inspection. <br /> Per the chef taro is peeled and shredded. <br /> All the vegetables are mixed with house made sauce. <br /> Then the mixture is placed in rice paper wrapper and deep fried. <br /> I observed packages of frozen rice paper wrappers stored in the freezer next to a box of frozen shrimp. <br /> Rearrange, and store raw shrimp separate from rice paper wrappers. <br /> Calamari is made by breading calamari with flour and then placing in the deep fryer. <br /> I observed breaded calmari that is stored in plastic ziplocks in freezer. <br /> Per, chef oil for deep frying is changed once a week. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Horacio, Chef <br /> EH Specialist: STEPHANIE RAMIREZ Phone: <br /> FA0000791 C00055060 SCO04 07/26/2022 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
