Santa Fe Road, Escalon/Riverbank, California <br /> Client Name: Peter Palomino <br /> Project Number: 618 Date: July 1, 2004 <br /> General Work Description: Advance up to five borings to approximately 30 feet bgs at various <br /> locations across site to investigate potential metals, petroleums, and VOCs in soil and <br /> groundwater beneath the site. <br /> SSP Prepared by: John Lane <br /> On-site Safety Officer: Joe Vasquez <br /> Project Safety Officer: John Lane <br /> This site safety and health plan (SSP) must be kept on-site and accessible to all on-site employees <br /> during all phases of site operations. The SSP must be reviewed and signed by all on-site <br /> employees, subcontractors, and visitors prior to entering the site. General safe work practices <br /> are identified in the Ground Zero Analysis, Inc. (GZA) Injury and Illness Prevention Manual <br /> and apply to all GZA work sites. <br /> The provisions set-forth in this SSP shall apply to GZA employees and any subcontractors <br /> working for GZA at the job site. All personnel working for GZA at the job site must read this <br /> SSP, and sign the attached Compliance Agreement before entering the work area. Field <br /> personnel may deviate from the safety provisions set-forth in this SSP, but only to upgrade or <br /> increase the safety requirements. GZA personnel may suspend work if unsafe working <br /> conditions are identified or observed, or unauthorized modifications to the safety provisions set- <br /> forth in this SSP are made. If changes in site or working conditions require changes in safety <br /> procedures, appropriate amendments to this SSP will be provided by the GZA project manager or <br /> supervisor, with the written approval of a GZA branch safety officer or the GZA Health and <br /> Safety Director. <br /> PAGE 2 <br />
