Ground Zero Analysis, Inc.
SITE SAFETY PLAN
Potential physical hazards include noise and physical proximity to drilling equipment, potential
methane gas, potential buried debris, fugitive dust emissions. A slight fire and explosion hazard
exists due to the potential presence of methane gas.
El Underground/Overhead Utilities (show location of utilities on site map)
✓ How were underground utilities identified? Underground Service Alert will be called at least
48 hours prior to arriving on site.
EXPOSURE MONITORING
Soil cuttings and breathing zone should be monitored for volatile organics using a photo ionization
detector (PID) when odors are detected. In addition, a Matheson Multi-gas Leak Detector Model No.
8067 IS will be utilized to monitor for the presence of methane gas in the boreholes. Intervals and
readings should be recorded in field notes. All sampling instruments will be calibrated per the
manufacturer instructions on a daily basis.
PERSONAL PROTECTIVE EQUIPMENT
As a minimum, Level D protection is required on all GZA worksites. Level D includes:
✓ Steel toed boots
✓ Safety Glasses
✓ Hard Hat
For each task on this project, identify additional protective garments as required and include the
conditions under which the level of PPE would be modified for each task.
Task(s) Condition Personnel Garment(s)
All Working Near Vehicle Orange Vests
All >20 ppm on PID for 5 1/2 Face Respirator
minutes
All >200 ppm on PID for 5 Evacuate Site Full Face Respirator
minutes
All >1,000 ppm on Matheson Evacuate Site
Multi-Gas Leak Detector
PAGE 4
